Definition
A urothelial carcinoma that arises from the bladder and is characterized by papillary urothelial proliferations and absence of invasion beyond the basement membrane. It displays either minimal cytologic atypia and infrequent mitotic figures that are limited to the lower half of the urothelium (low-grade) or marked architectural and cytologic abnormalities and frequent mitotic figures that are present at all levels of the urothelium (high-grade).
